Get a Library Card
Residents of Fairview City – may obtain a library card free of charge. Out of Town Patrons – residents of Sanpete County whose property taxes do not support the Fairview City Library will be charged an annual $12.00 user fee. Additional Library Cards – individual members of a family already holding a library card may obtain a card for a fee of $5.00 annually. This additional card fee applies to residents and not residents.
To obtain a library card, a patron needs to present proof of residency. Proofs of residency must show your physical address.
We will accept the following forms of proof of residency:
- Utility bills
- Rental agreements
- Bank/credit card statements
- Voter registration card
- Photo ID with current address on it
- Tax notice showing name and current address
- Name
- Address
- Phone number
- Birth date
When signing up for a library card, patrons will be required to provide the following information for their account:
For minors (0-17) parental information (name and ID) is required. Parents must also give consent to allow their children permission to use computers. By providing this information, patrons consent to our privacy policy, which ensures that their data is used solely for library services and is not shared without legal authorization.
Interlibrary Loan (ILL)
Interlibrary Loan (ILL) is a service that allows the library to provide material that is not available in our collection by borrowing the material from participating libraries and offering it to our patrons.
There is a $5 charge for each ILL item and a limit of 5 ILLs at a time. ILL no-show fee is $5.
